To Whom It May Concern,

Mountain Medical Center, LLC would like to inform you that Suzanne Haag, PA-C is no longer seeing patients in our clinic. She has taken a sabbatical from practicing medicine, and we wish her the best of luck in her next adventures.

At the same time, we would like to reassure you that your medical care can continue here at Mountain Medical Center. If this is your desire, you will need to be seen by either Dr. Joel Gates or Gabrielle (Brie) Silverman, PA-C for continuity of care.  We want to be sure that you get the best care we can offer. In order to do this, we will need to review your health history and care plan and get to know you individually.

If you would prefer to have your medical records forwarded to a different Provider, we are happy to assist you with this choice. For HIPPA privacy and security reasons, you would then need to physically sign a Release of Medical Information Form. We can provide you with that form here in the office, or you can find it on our website at http://ridgwaymountainmedical.com/files/MedicalRecordsForm.pdf. If you complete the form online, please fax the completed form to us at 970.626.9783 or mail it to 295 Sherman Street, Ridgway, CO 81432 so that we can forward your records to the appropriate Provider.

National Diabetes Month

Are you coming off the high of Halloween candy binges and coasting into the annual holiday season full of decadent plates full of comfort food like so many of us? If so, it is a great time to think about Diabetes, whether you know you have it or even if you are perhaps at risk of having this disease and not even know it.

More than 30 million US adults have diabetes, and at least 1 of 3 people will develop diabetes in their lifetime. If you have diabetes, your medical costs are probably twice as high as those for people without diabetes, and your risk of death is 50% higher. 

Risk factors include having prediabetes, being overweight, being 45 years or older, having a parent or sibling with type 2 diabetes, being physically active less than 3 times per week, and ever having gestational diabetes.

October is Breast Cancer Awareness Month

October is National Breast Cancer Awareness Month. One in seven Colorado women will be diagnosed with breast cancer in her lifetime.  Breast cancer remains the third leading cause of cancer death in Colorado, killing more than 500 women annually.  At the same time, there are more than 42,000 breast cancer survivors in Colorado.  Like many cancers, the likelihood of surviving breast cancer increases dramatically with early detection and treatment. Simply put, screening saves lives! Early detection of breast cancer dramatically increases women’s chances of survival – with nearly 98% of women surviving breast cancer as a result of receiving a preventative screening.
