To Whom It May Concern,

Mountain Medical Center, LLC would like to inform you that Dr. David Homer is no longer seeing patients in our clinic. Moving forward, he will be focusing his efforts at a practice in Farmington, New Mexico which he recently joined. We wish him the best of luck in his new endeavor.

At the same time, we would like to reassure you that your medical care can continue here at Mountain Medical Center. If this is your desire, you will need to be seen by either Dr. Joel Gates or Suzanne Haag, PA-C for continuity of care.  We want to be sure that you get the best care we can offer. In order to do this, we will need to review your health history and care plan and get to know you individually.

If you would prefer to have your medical records forwarded to a different Provider, we are happy to assist you with this choice. For HIPPA privacy and security reasons, you would then need to physically sign a Release of Medical Information Form. We can provide you with that form here in the office, or you can find it on our website at http://ridgwaymountainmedical.com/files/MedicalRecordsForm.pdf. If you complete the form online, please fax the completed form to us at 970.626.9783 or mail it to 295 Sherman Street, Ridgway, CO 81432 so that we can forward your records to the appropriate Provider.

Please know that we value your choice to receive medical services at Mountain Medical Center, and we are sorry for any inconvenience that this change may be causing for you.  If you have any questions about your care, please contact the office at 970.626.5123 so that we can assist you.
