We Are Accepting New Patients

To those in our community who are looking for a new medical home, we would like to make it clear that we ARE currently accepting new patients (and in fact, we have not been closed to new patients for many years).

There are only two caveats to that policy:
1) We are not currently accepting patients who live outside of Ouray County who have Medicaid insurance (however, we DO accept Medicaid insurance for all in-county residents).
2) If you are on an opioid prescription that is taken daily for pain control, we are probably not the best fit for you and would most likely be recommending you contact a pain management clinic for management of that medication.

In order to establish as a patient at our clinic, please stop our office by to fill out a new patient packet. You may also go to the forms section of our website and fill out the forms there. FYI, you can fill them out digitally but must have access to a printer to print and submit them. We need the following forms completed and submitted for review BEFORE new patient appointments will be scheduled.

1) New Patient Registration Form
2) New Patient Health History Page 1
3) New Patient Health History Page 2

Once a person is established as a patient here, we request their two most recent years of medical records and schedule provider appointments and lab work to handle any needed medication refills as appropriate. We can generally schedule new patient intake appointments within 1-2 weeks of receiving your completed paperwork, depending on the current health needs of our established patient population. Please do not wait until you are completely out of medication to take the steps to join our medical home.
